Bring the completed application and the following documents to the center nearest to your home to determine eligibility for our program:
Proof of Income, such as W-2 forms, pay stubs, written statement from employer, unemployment statement, award letter for TANF, award letter for SSI, child support statement. This is a federally funded program, and there are income guidelines that determine eligibility.
Child's Birth Certificate. Child must be a minimum of 3 years old by the school district's cut off date and cannot be kindergarten eligible.
If you need any assistance getting the required information or completing the application please contact the center in your neighborhood. The phone numbers and addresses are listed on the right side of this page.
At the center, a staff member will assist you in filling out the rest of the necessary forms to complete your application packet.
