Application Information The following information is needed to determine your childâ€™s eligibility for Head Start, Early Head Start or ECEAP: Initial Application Forms (Click to download) Application (English) Application (Spanish) Verification of Childâ€™s age Birth certificate, hospital certificate or medical coupon Verification of Income W2 form, pay stubs, TANF or SSI documentation for either the last 12 months or the last calendar year.
