Application Information The following information is needed to determine your child’s eligibility for Head Start, Early Head Start or ECEAP: Initial Application Forms (Click to download) Application (English) Application (Spanish) Verification of Child’s age Birth certificate, hospital certificate or medical coupon Verification of Income W2 form, pay stubs, TANF or SSI documentation for either the last 12 months or the last calendar year. Please return these items to: 1111 Cornwall Ave. Suite 200, Bellingham, WA 98225 If your child has special needs Please provide documentation such as a letter from a physician or therapist or a copy of medical or therapy reports.
