Eligibility/Apply Eligibility Monroe County Residency Meet family income requirements Application Process Parents fill out Pre-Registration Form If you qualify, PSFC sends you a letter with an appointment date During your interview the application is completed Please bring the following documents with you to your appointment: Your childâ€™s birth certificate Social Security Numbers for everyone in the household Your childâ€™s immunization record Your childâ€™s medical insurance card Your childâ€™s physical (within the past 12 months) Information pertaining to other services your child may be receiving (including: WIC, Easter Seals, IU20) If receiving Food Stamps bring confirmation letter from DPW.
