Medicaid or Other Health Insurance ♥
Current Income Tax Copies Or 4 Of Your Most Recent Check Stubs (if Parent is employed) ♥
Proof of TANF, grants & scholarships (if applicable) ♥
Pregnant women please bring documents with a heart. ♥
