Center-based Learning is for children between the ages of 3 and 5. These children participate in learning activities at the Head Start center during either a morning or afternoon session. Nutritional meals and snacks are provided each day. Meals are served family-style to teach children table manners. Children also learn to set tables and clean-up afterwards. To encourage personal hygiene, children wash their hands before each meal and brush their teeth after each meal. You also have the option of having your child picked up and dropped off by the Family Forum, Inc. school bus at no charge when your child is enrolled in the center-based program.

Who is Eligible?
Head Start generally serves children ages 3 to 5 who meet local eligibility requirements.
Pregnant women and
children from birth to 3 years of age are eligible for Early Head Start services. Children and
families who are homeless, in foster care, or receive TANF or SSI are also eligible for
services. Eligibility is determined by Head Start program staff and some families may be
eligible for services if they are determined to be at or below the federal poverty level. Some
grantees enroll a percentage of children from families with incomes above the Poverty Guidelines
as well.

How to Apply
Child Application
If you are interested in enrolling your child in any of the programs offered, please complete a child application and return it to Family Forum, Inc., 1507 Tower Ave., Suite 312, Superior, WI 54880. After the application is received, someone from the agency will set up a time to meet with you to do income verification to determine whether or not your child is income eligible for the program. A families income needs to be verified or documented as having zero income on the application according to the Poverty Guidelines. There are many ways, besides meeting the poverty guidelines, to determine if a child is eligible for Head Start. This will be determined at the time of verification.
