Minimum documents needed to complete an application:
Child’s CERTIFIED Birth Certificate or other State approved proof of birth.
Documentation of 4-6 WEEKS of the most current household income; recent pay stubs, 1040 tax form, SSD/SSI, employer letter, accountant’s statement, unemployment or other acceptable proof.
Proof of residency in Pasco County (FL ID, driver’s license, utility bill, lease agreement, etc.)
Additional documents if applicable:
Child’s social security card or number.
Information for other family members in the home (social security numbers and dates of birth).
Child’s health insurance card (Medicaid, Kid Care or Private Insurance); Proof of WIC

My six year old attend school at inprep academy and we live in south tampa and I wanted to enroll my 3 year old in Wesley chapel pasco county as well but I have no idea how to do so
