To be considered for the waiting list, you must fill out and return the following documents:Completed Intake Sheet (english version / spanish version)Verification of Income (at least one)Last pay stubOWF statement/FCDJFS benefit letterW-2 or 1040 Verification of Residence (at least one)Two current utility billsCurrent LeaseSettlement Statement Child's up-to-date Shot RecordChild's original birth certificate (If you don't have the original, you can get another one at the Bureau of Vital Statistics in downtown Columbus, located at 220 Parsons Avenue, 614-645-7331)IEP, if applicableThese forms can be submittted to any Head Start center or faxed directly to the Head Start main office 614-801-8443.
