2. THE CHILD’S IMMUNIZATION RECORD
3. PROOF OF RESIDENCY – any one of the following: Voter’s Registration, Driver’s License, Utility Bill or Telephone Bill
4. PROOF OF INCOME – any one of the following: Letter from Current Employer, Last Pay Stub, W2 Form, or TANF stub

Head Start and Early Head Start are federally funded programs that service primarily low-income children and their families. To be eligible for the Programs, families must qualify according to the income guideline set by the federal government. Eligible Head Start children must be 3 or 4 Years old by October 1, 2011 and reside in the City of Newark. Eligible Early Head Star
NPC is funded to serve 2504Head Start age and income eligible preschool children and their families. These children are served in 40 Centers and offer several program models in order to meet family needs.
