About / How to Apply
Enclosed is the application for Bergen County Community Action Partnership, Inc.,
Early Head Start/ Head Start Programs. Please complete all sections. You must personally
bring the completed application and required documentation to our Administrative Offices
located at:
263 Lafayette Ave.
Cliffside Park, NJ 07010
Applications are accepted:
Monday through Thursday
9:30 A.M until 11:30 A.M. and
1:30 P.M. until 3:30 P.M.
Required Original Documentation
1. The completed application.
2. Child’s original birth certificate with the raised seal, or passport.
3. An immunization record with dates of vaccinations given to your child.
4. Income verification for household:
• Last year’s Income Tax Return, Form 1040.
• Two (2) current consecutive pay stubs for bi-weekly or bi-monthly payroll; Four (4)
current consecutive pay stubs for weekly payroll.
• If you are receiving any form of public assistance, we will need a letter from Social
Services stating what and how much you are receiving
These programs are funded by the U.S. Department of Health and Human Services, and provide services to 501 low-income infants, toddlers, pre-school children and their families. All classrooms run from 8:45 am to 3:45…
