Enroll your child Who is eligible: Reside in Clay, Platte or Jackson counties in Missouri Head Start available in all three counties Early Head Start only available in Jackson county. Have an income that is at or below 130% of federal poverty guidelines What to bring to enrollment: Proof of income (W-2 forms, paycheck stub or letter from employer) For foster children - letter from DDS stating income Proof of medical insurance Physician and dentist information (name, address, and phone number) Child's immunization records, physical exam, and dental exam Child's birth certificate or baptismal documentation To enroll your child Applications are accepted yearlong.
