Head Start Application and Enrollment Information The required application for Head Start services may be down loaded here,or you may call Head Start at 763.783.4300 and ask for an application to be mailed to you.(To view this application, you must have Adobe Acrobat Reader installed on your computer. This can be downloaded free by visiting the Adobe website at http://adobe.com .) Please print this application and complete all information requested on the form; sign and date the form. You must also include an income statement reflecting your current status. Return the forms to the office on the top of the application form. You will receive a letter notifying you of your eligibility. If you have any questions or need help completing the application, please call 763.783.4300, TTY 763.783.4724, and our receptionist will help you. Registration is on-going.
