To Apply for Early Head Start Call Early Head Start at 517-372-9411. Age Guidelines: Early Head Start serves pregnant women and children through the age of 3. To Enroll If youâ€™re application has been accepted, a Family Advocate will contact you to schedule an enrollment appointment. Please bring the following documentation along with you to your appointment (The following information pertains to the child being enrolled and the parents/guardians): Proof of Income Social Security numbers Immunization record of child Childâ€™s latest physical (MUST be within the past 12 months) Verification of birth of child (Birth Certificate or Hospital Certificate) Dental exam performed by a licensed dentist MUST be completed within first 90 days of childâ€™s enrollment
