How to Apply for Enrollment
Step 1
Download the Preliminary Enrollment Application (Adobe Acrobat Reader is required to open document). (go to: Head Start Preliminary Enrollment Application Form): http://hss.sbcounty.gov/psd/enrollment/HeadStartGenisisEnrollmentApplication.pdf
An intake appointment in order to complete the application process may be made by returning the completed preliminary enrollment application form to:

An Eligibility Worker in your area will contact you and schedule an appointment at a convenient time for you. The Eligibility Worker will remind you as to what you need to bring to your appointment in order to expedite the application process.

Once your appointment has been scheduled, please bring the following to your appointment:

Verification of Family Income
Medi-Cal
