About / How to Apply

We will be accepting applications next on September 18th, from 9:00 a.m. to 1:00 p.m. Come, get registered and get in the "Waiting Pool"! Children need to be 3 by September 1, 2012 to be considered for enrollment this year. Bring the following with you: Certified Birth Certificate Social Security Card Current Immunization form (blue form from the Russell County Health Dept.) verification of income (ex.-Check stub, Child Support, social Security Benefits letter, etc...) Medicaid card or proof of insurance if applicable proof of residency (ex.- utility bill, water bill, copy of your lease - if you live with someone else, you will need to ask for a Third Party form for that person to complete and to be notarized. (The proof of residency can NOT be a phone or cable bill.) Approved Transfer form from the Phenix City Board of Education if you live outside the Phenix City School District. Lee County residents are ineligible. Each child considered for enrollment needs a completed
